Professional guidance is not a letter entitlement

Bring context rather than a script
Explain why the document is being requested, how your difficulties affect daily life, and what role the animal has. Bring the actual housing form if one exists. A professional needs your circumstances, not a memorized list of qualifying symptoms.
For a remote visit, confirm authorization for your location. Ask how the office handles correction of factual errors, additional forms, and later verification. Those practical service details are worth clarifying before payment.
Separate the housing decision
A clinician’s document, if appropriate, can be part of an accommodation request. Identify the rule you seek to adjust, send the request to the property’s accommodation contact, and retain the response. Do not submit a template bearing a professional’s name without their review and authorization.
