A sample cannot create a clinical opinion
An emotional support animal letter template can show a document’s general structure. It cannot establish a professional relationship, verify a disability-related need, or supply a clinician’s judgment. Filling in a free PDF does not make it equivalent to documentation issued after an assessment.
For that reason, this page does not provide a fill-in medical certificate. If a clinician determines that documentation is appropriate, they should create and authenticate it within their professional practice. Use our document checklist to review factual details afterward.
What you can write yourself
You can write the message that accompanies a housing request. Its purpose is to explain the accommodation you are seeking and start a clear exchange. It should describe your circumstances accurately without impersonating a medical professional.
The following is an illustrative resident message. Adapt it only where its statements are true. The example does not determine whether your situation qualifies under any particular law.
An accommodation request example

I live at [address/unit]. I am requesting a disability-related accommodation involving my animal, [animal’s name and type]. The policy adjustment I am requesting is [describe the specific adjustment].
[If accurate: I have attached documentation from my healthcare professional relevant to this request.] Please let me know the contact handling accommodation requests and whether you need clarification about the information provided.

Before you attach any paperwork
Check that your name and the intended recipient are correct. Keep the clinician’s file intact. Do not edit a signature, change an issue date, reuse another person’s documentation, or add clinical statements yourself.
Ask the issuer how verification requests should be handled. A housing contact may need limited information relevant to the request; that does not mean you should automatically send your full treatment record. If you are unsure what is appropriate, seek guidance on your particular circumstances.
Why the request still needs a review
Even careful wording and authentic documentation do not guarantee acceptance. The housing provider must address the particular request under the rules that apply. Read the housing update and the relevant state guide before relying on older sample letters found online.
