Identify what the word free describes
A free PDF sample demonstrates a document’s layout. A free screening form collects information. Neither is automatically a completed professional assessment or a letter issued to you. Before entering personal details, find out what happens after the free step and whether payment is required for the appointment.
Do not place your name and a clinician’s signature on a downloadable example. If you need a sample request to write in your own voice, use our housing request template.
Ask about care you already receive
If you have a treating professional, ask whether assessing the need and providing appropriate documentation would be part of existing care or billed separately. There is no universal promise that a doctor will write an ESA letter free of charge.
For a new service, ask about sliding-scale appointments, financial assistance, and the complete price where available. Verify the service terms directly. A very low headline price may apply only to a document add-on rather than the clinical appointment.
Compare the total cost
Write down appointment cost, documentation if appropriate, extra-animal charges, additional forms, verification support, and future follow-up. Mark any item the provider has not explained. Compare that total with your actual need instead of judging by a coupon alone.
If insurance or an employer benefit is involved, ask the plan and provider before assuming coverage. Avoid buying an assessment solely because a sales page promises a particular amount of future rent savings. The clinical outcome and the housing decision are separate, and neither should be budgeted as guaranteed.
