These universities publish animal or housing accommodation information. The notes below identify a starting point; they do not reproduce complete policies or guarantee eligibility. Verify current forms, deadlines, and instructions before submitting a request.

University housing approval and a clinician’s documentation are separate steps. Keep both in your move-in plan.

Prepare one organized submission

Record the office, form, deadline, supporting document, and confirmation for your institution. Ask whether additional animal records or a care agreement are part of the process. A provider receipt or screening result does not complete these tasks.
